The informed consent is an ethical and legal requirement for potential participants to enroll in a study. There is ample of evidence that understanding consent information and enrollment is challenging for participants in clinical trials. On the other hand, the reasoning process behind decision-making in HIV clinical trials remains mostly unexplored. This study aims to examine the decision-making process of people living with HIV currently participating in antiretroviral clinical trials and their understanding of informed consent. We conducted a qualitative socio-constructivist (...) study using semi-structured interviews. Eleven participants were selected by purposive sampling in Argentina until data saturation was reached. A content analysis was performed. The findings highlight the fact that some participants decided to enroll on the spot, while others made the decision a few days later. In all cases, the decision was based on different aspects of trust (in doctors, in the clinical research site, in the clinical trials system) but also on emotions associated with HIV and/or treatment. Moreover, while people living with HIV felt truly informed after the consent dialogue with a researcher, consent forms were unintelligible and unfriendly. The immediacy of patient decision-making has rarely been described before. Enrollment in an HIV clinical trial is mainly a trust-based decision but this does not contradict the ethical values of autonomy, voluntariness, non-manipulation, and non‐exploitation. Thus, trust is a key issue to be included in reshaping professional practices to ensure the integrity of the informed consent process. (shrink)

This volume brings together a collection of ten original essays which present new analyses of social and relational equality in philosophy and political theory. The essays analyze the nature of social equality and its relationship with justice and with politics.

This chapter serves as an introduction to the collected volume. In the first section, we aim to provide background on important themes in social egalitarianism and to set the context for understanding which significant questions the chapters in this book pose and attempt to answer. In this section we focus especially on what could be said to characterize socially egalitarian relationships, on which relationships are of concern, and on what might make social egalitarianism distinct. Much research is currently being conducted on health care practitioners' experiences of moral distress, especially the experience of nurses. What moral distress is, however, is not always clearly delineated and there is some debate as to how it should be defined. This article aims to help to clarify moral distress. My methodology consists primarily of a conceptual analysis, with especial focus on Andrew Jameton's influential description of moral distress. I will identify and aim to resolve two sources of confusion about (...) moral distress: the compound nature of a narrow definition of distress which stipulates a particular cause, i.e. moral constraint, and the distinction drawn between moral dilemma and moral distress, which implies that the two are mutually exclusive. In light of these concerns, I argue that the definition of moral distress should be revised so that moral constraint should not be a necessary condition of moral distress, and that moral conflict should be included as a potential cause of distress. Ultimately, I claim that moral distress should be understood as a specific psychological response to morally challenging situations such as those of moral constraint or moral conflict, or both. What kind of equality should we value and why? Current debate centres around whether distributive equality is valuable. However, it is not the only (potentially) morally significant form of equality. David Miller and T. M. Scanlon have emphasised the importance of social equality—a strongly egalitarian notion distinct from distributive equality, and which cannot be reduced to a concern for overall welfare or the welfare of the worst-off. However, as debate tends to focus on distribution, social equality has been neglected and (...) we do not have a clear understanding of what it is and why it might be valuable. Teaching and learning in a virtual learning environment poses some difficulties, but also challenges and opportunities to rethink the whole learning process, particularly in abstract subjects like logic or high level mathematics. On the other hand, resources and ways to work, now available in VLEs, might soon extend to all kinds of environments. In this paper, we will present experiences at the Open University of Catalonia , a particular VLE, concerning the whole process of teaching logic and mathematics. Schillers Anthropologie und die Philosophische Anthropologie – dahinter stehen zwei Denksysteme mit zahlreichen Parallelen. Während den Theorien selbst in den letzten Jahrzehnten große Aufmerksamkeit geschenkt wurde, ist die Analogie zwischen ihnen bislang nur wenig erforscht. Die Studie nutzt Schlüsselbegriffe und konstante Denkfiguren, die aus den Werken Schelers, Plessners und Gehlens herausgearbeitet und vor ihrem wissenschaftlichen wie gesellschaftlichen Hintergrund erklärt werden, zu dem heuristischen Zweck einer Neuinterpretation der schillerschen Philosophie vom Menschen. So deckt der typologische Vergleich zwischen den Schriften des späten (...) 18. und des frühen 20. Jahrhunderts strukturelle Parallelen ihrer gedanklichen Konfliktsituationen auf und macht Schillers Theorie vom Menschen als spezifische Philosophie der Moderne begreiflich, die auf die Krisenerfahrungen ihrer Epoche mit einer ästhetischen Anthropologie der Freiheit unter der Bedingung menschlicher Lebendigkeit antwortet. Auf diesem Wege eröffnen sich neue Perspektiven, durch die der Anthropologe Schiller gegenüber dem 'Moraltrompeter von Säckingen' ins rechte Licht rückt und Spannungen seines Denkens, wenn auch nicht aufgelöst, so doch verständlich gemacht werden. The origin and evolutionary dynamics of the spatial heterogeneity in genomic base composition have been debated since its discovery in the 1970s. With the recent availability of numerous genome sequences from a wide range of species it has been possible to address this question from a comparative perspective, and similarities and differences in base composition between groups of organisms are becoming evident. Ample evidence suggests that the contrasting dynamics of base composition are driven by GC‐biased gene conversion (gBGC), a process (...) that is associated with meiotic recombination. In line with this hypothesis, base composition is associated with the rate of recombination and the evolutionary dynamics of the recombination landscape, therefore, governs base composition. In addition, and at first sight perhaps surprisingly, the relationship between demography and genomic base composition is in agreement with the gBGC hypothesis: organisms with larger populations have higher GC content than those with smaller populations. Injury to the central or peripheral nervous system is often associated with persistent pain. After ischemic injury to the spinal cord, rats develop severe mechanical allodynia-like symptoms, expressed as a pain-like response to innocuous stimuli. In its short-lasting phase the allodynia can be relieved with the [gamma]-aminobutyric acid (GABA)-B receptor agonist baclofen, which also reverses the hyperexcitability of dorsal horn interneurons to mechanical stimuli. Furthermore, there is a reduction in GABA immunoreactivity in the dorsal horn of allodynic rats. Clinical neuropathic (...) pain of peripheral and central origin often cannot be relieved by opiates at doses that do not cause side effects. The loss of sensitivity to opiates may be associated with the up-regulation of endogenous antiopioid substances, such as the neuropeptide cholecystokinin (CCK). CCK and its receptor (CCK-R) protein is normally not detectable in rat dorsal root ganglion cells. After peripheral nerve section, both CCK and CCK-R are up-regulated in the dorsal root ganglia. Furthermore, CI 988, an antagonist of the CCK-B receptor, chronically coadministered with morphine, reduces autotomy, a behavior that may be a sign of neuropathic pain following peripheral nerve section. Thus, opiate insensitivity may be due to the release of CCK from injured primary afferents. Similarly, in the chronic phase of the spinal ischemic model of central pain, the allodynia-like symptom is not relieved by systemic morphine, but is significantly reversed by the CCK-B antagonist. Consequently, up-regulation of CCK and CCK-R in the CNS may also underlie opiate drug insensitivity following CNS injury. Thus, dysfunction of central inhibition involving GABA and endogenous opioids may be a factor underlying the development of sensory abnormalities and/or pain following injury to neural tissue. Just as bodily actions are things you do with your body, mental actions are things you do with your mind. Both are different from things that merely happen to you. Where does the idea of mental action come from? What are mental actions? And why do they matter in philosophy? These are the three main questions answered in this paper. Section 1 introduces mental action through a brief history of the topic in philosophy. Section 2 explains what it is to (...) be a mental action in terms of intentional action. Section 3 argues that the fact that we can perform mental actions has some striking consequences concerning mental content. Section 4 shows how mental action helps to explain some important facts about self-knowledge and the normativity of thought. Many great authors claim that reading literature can expand your phenomenal imagination and allow you to imagine experiences you have never had. How is this possible? Your phenomenal imagination is constrained by your phenomenal concepts, which are in turn constrained by the phenomenology of your own actual past experiences. Literature could expand your phenomenal imagination, then, by giving you new phenomenal concepts. This paper explains how this can happen. Literature can direct your attention to previously unnoticed phenomenal properties of your (...) own past experiences, as brought to mind by involuntary memory. This process gives you the opportunity to notice those phenomenal properties for the first time, and so also to form phenomenal concepts of those properties. Forming a new phenomenal concept strictly expands the range of your phenomenal imagination. It gives you the capacity to imagine more than you could have before, including certain specific kinds of experience that you yourself have never enjoyed firsthand. Literature’s capacity to expand your phenomenal imagination in this way is central to its social, moral, and epistemic value. (shrink)

Progress in transplantation outcomes depends on continuing research into both donor and recipient factors that may enhance graft and patient survival. A system of licencing for transplantation research, introduced by the Human Tissue Act 2004, which separates it from the transplantation process (then exempt from licencing), has damaged this vital activity by a combination of inflexible interpretation of the 2004 Act and fear of criminal liability on the part of researchers. Now, following the European Union (EU) Directive (2010) on standards (...) of quality and safety of human organs intended for transplantation, new UK Regulations have been drafted, which are intended to implement it. These Regulations impose a compulsory licencing system, similar to that for research, on the whole transplantation process. This goes beyond what is required by the Directive and may even have an inhibitory effect similar to that already seen in research. Initial draft Regulations went further, imposing criminal sanctions for breaches. However, following a public consultation process, the Department of Health (DH) has recently stated that, as a result of the overwhelming view of respondents that the proposed licencing system was unnecessary, all sanctions under the final Regulations are now decriminalised, with the sole exception of operating without a licence. While this does not eliminate the negative effect of licencing, it does suggest an awareness of the DH that excessive regulation unnecessarily harms the transplantation process. An opportunity thus arises for the Human Tissue Authority (the regulatory body for both the new licences and research licences under the 2004 Act) to end the current illogical and harmful separation of transplantation and transplantation research by ensuring that all centres licenced for organ donation, retrieval and transplantation are also fully licenced for related research. This essay will consider the dispute between Charles Sanders Peirce and John Dewey on the nature and validity of the theory/practice distinction. Their dispute concerns whether a strong affirmation of this distinction enables scientific inquiry or disables it. Inherent in this issue are two related questions, namely whether tmth can be pursued for its own sake, and, as a subset of this question, whether there is an evidence proper to the consideration of pure possibilities. I will argue that there is (...) a common ground between the logic of inquiry which Dewey is determined will have practical effect, and the logic of inquiry which Peirce is concemed will discover the truth promised by correct investigation. This compromise is found in the philosophy of Bemard Lonergan, whose critical realism includes an account of concrete judgement strikingly similar to that of John Dewey, but whose account of the normative stmcture of cognition, emanating from the individual's radical desire to know, also incorporates Peirce's emphasis on the pursuit of truth and on the validity of the scientist's consideration of pure logical possibilities. I will present these similarities, and will consider the stmcture of Einstein's 1905 paper, "On The Electrodynamics of Moving Bodies," as a case study which supports Lonergan's (and Peirce's) account of cognitional stmcture, but which poses problems for Dewe/s instrumentalism. The essay concludes that Lonergan offers a critical realism which successfully renounces the 'spectator' theory of knowledge without underestimating the importance of the normative element of understanding in the human desire to know. The adaptation-by-binding account and the arousal-biased competition model suggest that emotional arousal increases binding effects for transient links between stimuli and responses. Two highly-powered, pre-registered experiments tested whether transient stimulus-response bindings are stronger for high versus low arousing stimuli. Emotional words were presented in a sequential prime-probe design in which stimulus relation, response relation, and stimulus arousal were orthogonally manipulated. In Experiment 1 (N = 101), words with high and low arousal levels were presented individually in prime and probe displays. (...) In Experiment 2 (N = 170), a high arousing affective word was presented simultaneously with a neutral word during the prime display; in the subsequent probe display, either the arousing or the neutral word repeated or a different high versus low arousal word was shown. Data from both experiments did not demonstrate a modulation of SRBR effects by stimulus arousal and SRBR effects were of equal magnitude for word stimuli of high and low arousal levels. These null results are not in line with binding accounts that hypothesise a modulatory influence of emotional arousal on perception-action binding. (shrink)

Aesthetic value empiricism claims that the aesthetic value of an object is grounded in the value of a certain kind of experience of it. The most popular version of value empiricism, and a dominant view in contemporary philosophical aesthetics more generally, is aesthetic hedonism. Hedonism restricts the grounds of aesthetic value to the pleasure enjoyed in the right kind of experience. But hedonism does not enjoy any clear advantage over a more permissive alternative version of value empiricism. This alternative is (...) aesthetic liberalism. On this view, an object’s aesthetic value is fully grounded in any value—not just the hedonic value—of a correct and complete experience of its sensory features. To demonstrate the advantages of liberalism over hedonism, I apply both views to analyze the aesthetic value of the Spring Temple Buddha and Anselm Kiefer’s Seraphim. I detail four advantages of liberalism over hedonism, and I conclude by defending liberalism from two kinds of objection. (shrink)

Locke claims that we have sensitive knowledge of the external world, in virtue of the fact that simple ideas are real, true, and adequate. However, despite his dismissive remarks about Cartesian external-world skepticism, Locke gives us little to go on as to how knowledge of the external world survives the fact of perceptual error, or even how perceptual error is possible. I argue that Locke has an in-principle problem explaining perceptual error.

In the first half of the book, Pyle lays out Malebranche's two key doctrines as responses to tensions within the Cartesian accounts of causation and of ideas. Previous research has shown that stimulus–response (SR) binding and retrieval processes also occur when responses are only observed in another person ( Giesen et al., 2014 ). Importantly, this effect depends on the two individuals interacting interdependently during the task (e.g., competition or cooperation). Interdependence, however, must not necessarily result from task-related demands, but can also reflect an intrinsic feature of a given relationship. The present study examines whether observing responses of one’s romantic partner also produces stimulus-based retrieval of observed (...) responses even if the task itself does not involve interdependence. Participants performed a task pairwise, either with their romantic partner or with a stranger. In a sequential prime-probe design, both participants of a pair gave color responses themselves (actors) or merely observed these (observers) in alternating fashion. As expected, stimulus-based retrieval of observationally acquired SR-bindings occurred only in romantically involved pairs; participants interacting with a stranger showed no retrieval effects. We conclude that mental representations of self and other are more closely intertwined in romantic couples, which produces automatic retrieval of observationally acquired SR binding effects even independently of the task itself.
